Transitional Shelter Interest Form

This is a 12 month, clean and sober, program.  Program participants will be required to meet at least monthly with our case manager to work on goals and housing.  Everyone is required to do a weekly chore, attend a weekly meeting, take random drug tests, and abide by all program expectations.

The intake process involves an application, a background check, and an interview.  It could take a couple weeks to get through the process.
